Upload Registration Documents

When all required documents have been uploaded and reviewed by the NPDB you will receive an email notification. Please allow the NPDB one business day to process your documents.

The registration document may be more than one page, and if so, both pages are required. Please note that all documents must be completed in order to process your registration.


To ensure you receive email notifications from the NPDB, please make sure you have added the NPDB to your "Safe Senders List." A "Safe Senders List" is a trusted email address and/or domain name list that an Internet provider keeps that allows emails to be sent to your inbox. Addresses on this "Safe" list will always be delivered and never be treated as junk email or spam. To ensure delivery of email from the NPDB, add the npdb.hrsa.gov domain to your "Safe Senders List."
