Select Send me a password to get a one-time password sent to your phone.

Please wait ...

Enter the 6-digit password that we sent to your phone to sign in. You can only use it once.

If you have not signed in for 60 days you will be prompted to reactivate your account by XMS/AMS and NPDB.

Users Must Complete Identity Verification

To meet the highest cybersecurity standards, the NPDB requires users to verify their identity.

If you have a Social Security number or individual taxpayer identification number, you must use it to verify your identity. If you do not have either number, you must verify your identity with a valid passport. Visit ID.me's help page exit icon for instructions on verifying with a non-U.S. passport.

You can only have one verified ID.me account. Visit our What do I do if I have a duplicate ID.me account FAQ for help.
